2007 Girls Camp Deadlines

I apologize for the lateness of this letter,  but we still have not been able to access the forms on the website to change the information.  So we’ll just go with last years’ forms.  You should all be able to use your passwords from last year to access the forms and rosters to fill them out.  If your passwords do not work or you do not have them, let me know ASAP so we may try to find another solution

The camp charges on the voucher will be incorrect.  The state presidents have agreed that the final camp charges will be $400 per player, $400 per coach/admin/adult, $300 for the U19s.

For your state team players, we will only require one original, notarized medical release form, the medical history form with copy of the insurance card, the player data form, and the signed code of conduct form. Do not include social security numbers on any forms unless it is on the medical insurance information. Some companies still use that as identifiers.

 We will use the following as deadlines:

 Camp numbers—players and adults—male or female—TODAY

If your DOC is attending camp but not as a team coach, how long will he stay?

 JUNE 1—travel information emailed to R3GODP@aol.com

  If you’re flying, give us the flight numbers for both arrival and departure and how many people will be on the flight.

  If you’re arriving by buses, give us the approximate arrival time.

  If your players are arriving individually, please let them know they may not check-in until the team administrator arrives.

  Don’t forget that the all camp meeting will begin at 4:00 on arrival day with the first training sessions to begin at 5:00 or 5:30.

 JUNE 1-proof of a current criminal background check for all accompanying coaches/admins/adults.  No persons will be allowed in camp without certification from your state office by this deadline.  There is a form on the website on which to enter that information 

 NEW: Please have your state president or their designate sign this form attesting to the validity of the information.  Email this to R3GODP@aol.com

If you’re sending all the paperwork in electronic format, email to R3GODP@aol.com.

Then, all you’ll need to bring to camp are the original medical release forms and any roster changes.
